We are seeking compassionate and skilled Licensed Vocational Nurses (LVNs) to join our team in delivering continuous care to patients in need of critical symptom management and support. This position involves close monitoring, skilled interventions, and emotional support to ensure the patient’s comfort and the family’s peace of mind.
Key Responsibilities:

Provide bedside nursing care during assigned continuous care shifts, ensuring patients’ comfort and managing symptoms effectively.

Monitor patients’ conditions closely, documenting vital signs, symptoms, and any changes in status.

Administer medications as prescribed and perform treatments such as wound care, catheter care, or oxygen therapy, as indicated in the Plan of Care (POC).

Communicate changes in the patient’s condition to the RN Case Manager, Physician, or Director of Nursing (DON).

Educate patients and families on symptom management, medication administration, and hospice services.

Maintain accurate, detailed, and timely documentation in the electronic medical record (EMR).

Provide emotional and psychological support to patients and their families during critical times.

Ensure compliance with hospice policies, procedures, and state and federal regulations.

Collaborate effectively with the interdisciplinary team to ensure high-quality, patient-centered care.
Qualifications:

Active LVN license in the state of Texas.

Minimum of one year of nursing experience, preferably in hospice, home health, or palliative care.

Strong clinical skills with a focus on symptom management and patient comfort.

CPR certification.

Excellent communication and interpersonal skills.

Ability to work independently and as part of an interdisciplinary team.

Proficiency in electronic medical record (EMR) systems is preferred.

Reliable transportation and valid driver’s license.
Schedule and Compensation:

Flexible schedules available, including day, night, and weekend shifts.

$26 per hour.
